Please sign-up prior to training. You may complete a sign-up form regardless of enrollment to ensure that you receive news and updates. The sign-up form contains personal contact information, emergency contact information, general liability waiver, and photo release, it will also enter you into our database. If you are signing up on behalf of your family member, just note that an individual sign-up is required for every student/family member that will be training with us.

• Select “Visitor” if you are just trying things out, not sure about a membership plan yet, or if you attend another school but would like to participate in our open mats. See example form for reference.
  

• Select “Member” if you are enrolling in a membership and are ready to join the team. Once you input your information you can select a membership plan right from the sign-up form - or email info@shogunwestbjj.com with you enrollment selection(s). See example form for reference.

  Note: payment is not necessary until membership enrollment option is selected.
